I hereby enroll in the 2010 Dorothy Hamill Figure Skating Fantasy Camp and agree to the terms and conditions as outlined below. I agree to pay $3850 ($3650 for early registration). $500 is enclosed as deposit and due upon registration. The second payment is due by March 22nd and the balance is due by July 22, 2010. Any money paid will be refunded in full on cancellations before March 22, 2010, 50% refunded on cancellations prior to July 22, 2010. Money is non-refundable with cancellations after July 22, 2010.

My signature indicates that I understand there are inherent risks in the sport of figure skating and there are risks of bodily injury from my participation in the activities offered by Dorothy Hamill Figure Skating Fantasy Camp and that I agree to release any claim I may have against, and, further agree to indemnify and to hold harmless in all respects, the camp, the camp owners, directors, staff, coaches and other employees with regard to any claims for injuries or damages that may be incurred as a result of participation in said activities.

This application shall be construed and governed by the laws of the state of Massachusetts. Any suit resulting from a dispute arising hereunder shall be pursued by either party only in the appropriate superior court in the county in Massachusetts where the camp is located, or if jurisdiction will so permit, in the Federal District Court. The camp and the undersigned consent to venue and personal jurisdiction in such courts.
